If you are an existing patient and you refer, you both get entered. Or, if you are a new patient and receive a full exam, cleaning, and eligible x-rays.

Must be 21 years of age or older to enter. The potential winner will be selected randomly on the ending date (as noted in the contest application) from among all the eligible entries received. Prize must be accepted as awarded. The prize may not be sold, transferred, and is not convertible to cash.
